Enter a Connection Code, provided by our staff, and you will have access to a personal caregiver webpage. Login whenever you want to post new information about your loved one and the status of his or her care.
Use email to tell as many (or as few) people as you want about your Care Connection page. After that, they may check in as often as they want to find out what is going on with the patient, the family and what you may need. Anyone who checks into the website can even leave a message of encouragement if they wish.
