Jonathan A. Cholewinski

Jonathan A. Cholewinski, age 15, of Fond du Lac, died Thursday, June 23, 2016 at Children’s Hospital of Wisconsin. He was born on April 5, 2001 the son of Theodore and Diane (Chapin) Cholewinski. Ted chose Jonathan as his name because in Hebrew it means “God’s gracious gift”. Ted was prophetic in that choice as Jonathan was truly a gift and brought a smile to everyone’s face.



SURVIVORS: his father Ted of Fond du Lac; three sisters, Carrie Gau, Shelley Lancaster, and Colette Taylor; his godmother, Joanne Sheridan; his godfather, John Vandecastle; Theresa Leppla NP; and other relatives and friends. He was preceded in death by his mother Diane. Special thanks to everyone that has helped with Jonathan over the years, you are all family.



Visitation will take place on Saturday, July 9, 2016 from 9-11 A.M. at Holy Family Church-Holy Family, 271 4th St. Way, Fond du Lac, WI, 54935. A Funeral Mass will follow at 11:00 A.M. at the church. Cremation has taken place and interment will follow the service at Chapel of the Risen Christ Mausoleum, Calvary Cemetery.



In lieu of flowers, memorials may be directed to Holy Family Church of Children’s Hospital of Wisconsin.
