Phyllis D. Cook

Phyllis Dian Cook, 84, of Fond du Lac, died Thursday, June 18, 2026 at Hospice Home of Hope. She was born October 11, 1941 in Illinois, the daughter of Kenneth and Dorothy (Gallup) Horton. On May 7, 1998 Phyllis married Ronald Cook, Sr in Las Vegas. She worked for Wells Manufacturing until her retirement in 2007. Phyllis enjoyed arts and crafts, boating, fishing, playing Bingo and computer games.

She is survived by three children, Allen (Catherine) Quinn, Debbie (David Novitski) Quinn, and Stephanie (Richard) Roth; grandchildren, Steven (Elizabeth) Quinn, David (Kristin) Quinn, Jenna Roth, and Rebecca Roth; step grandchildren, Brian Novitski, Timothy Novitski, Kevin Novitski, and Anastacia Roth; great grandchildren, Aubrey, Avril, AJ, Viola, Leon, Kye, Ivy, Charlee, and Rucker; and other relatives. She was preceded in death by her parents; her husband, Ronald; brother, Omer Horton; and sister, Patricia.

The visitation will be held Thursday, June 25, 2026 from 9:30 – 11:00 AM at Uecker-Witt Funeral Home. The funeral service will follow at 11:00 AM on Thursday at the funeral home. Burial will follow in Ledgeview Memorial Park Cemetery.

In lieu of flowers, memorials can be made to the family to be given to Hospice Home of Hope in Phyllis’ name.
